WARNING: Use HTML copy at your own risk. Latex2html
utility, which was used to translate Latex version into html,
could not cope with this document. Conversion problems that I've spotted
include (1) random reordering of equations and artwork,
(2) spurious vertical and horizontal black lines in graphics files,
(3) some images are erroneously shaded grey, (4) certain mathematical symbols, e.g. $\mathcal{T}$,
were not converted at all, and appear as gibberish alfanumeric references, to name but a few.
